CJON St. John's & CIHF-TV-2 Saint John
According to postings on Digital Home, these channels are now over-the-air in digital.
CJON's analog transmitter shut down July 11, 2011 in preparation for the digital conversion.
CIHF-TV-2 is a flash cut, remaining on channel 12.

All Planned Calgary-DT Transmitters On-Air and on Post-Transitional Channels
As of today, the following two Calgary, AB transmitters have flashcut to digital:
CJCO-DT
CKCS-DT
With those two transmitters converted, all digital transmitters planning to go digital in this market are now broadcasting in digital on their post-transitional channel allotments. Note that two transmitter,s CBRFT and CIAN are not transitioning to digital and will have to shut down on August 31, 2011 unless authorized otherwise by the CRTC.

Two Ottawa updates:
CICO-TV-24 shutdown
CICO-DT-24 in service
CIII-TV-6 shutdown
CIII-DT-6 inservice, annoyingly with virtual channel 4.1 instead of 6.1. (4.1 is already in use in this market)

What's the reasoning behind the use of 4.1?

We're assuming they made a mistake, and that they'll hopefully fix it soon.
I don't have TSReader (my computer tuner is on a mac), so I don't know what they're really sending. Some people are seeing 6.1, but I think that depends on how individual tuners deal with virtual number conflicts. 4.1 is used by the first local DT station in the market, CBOT-DT (RF25).
I wouldn't put it in the database as 4.1, unless you don't mind switching it back to 6.1 (hopefully) in a few days when they fix it.
Any idea when the coverage plots will be ready for CIVO, CHRO, and CHCH?

CHCH Hamilton switched off its analog signal and went to 11.1 digital. According to Wiki they reduced transmitting power from 60kW to 6 kW

CBMT DT in Montreal moved from 20 (6.1) to 21 (6.1) where I believe it will stay.

I forgot to follow up earlier -- Global fixed the virtual channel number issue within a day or so, but a complete rescan was required, so I didn't see it right away. Most of Global's other PSIP issues have been cleared up. There reamins the issue of the clock running 15 seconds slow over the air, although it is set correctly at Master Control. This also affects at least three other stations in the market.
