Interesting antenna. Looks like it is for VHF real channels 2-13. Most of your channels are UHF with the exception of WKRC on CH 12. You would need something like an RCA ANT751 or a Winegard HD7694P, no amp.

You have some very strong FM signals that might cause interference. I don't have your exact address, so I used your short coordinates to make an FMFOOL report. Insert an FM trap, or 2 in series.

You would probably connect the balun to the center of one of the long elements (maybe the middle one), where there should be 2 wing nuts. I would need a closeup to tell for certain.

Even if your antenna doesn't look like it's the right one for your signals, you should try it first if you can get to it easily.
