The Winegard HD 7080P antenna with no amplifier should receive most of the stations on your report. It is Low VHF for your 4 and 5. High VHF for your 12 and 13, and Uhf for the rest. You should not use an amplifier. For your rotor I recomend the Aspen Eagel Roto 100. With this rotor you can use the signal coax to tun the rotor. It has a 360 degree read out, so you can aim the antenna to magnetic North when installing. This will allow you to locate the channels by turning to the degree number that matches your report.

Run the rotor from end to end before installing. Use solid copper RG6 coax. If hooking up multipul sets, split after your coax exits the control box, or use a splitter that has a DC pass port to feed the control box.
