It looks like a PCB (Printed Circuit Board) balun, which matches the antenna to the RG6 coax downlead.

What make and model is the antenna?

Whats is this called? It broke on my uhf antennaIf that is from an Antennas Direct 91XG, they have a lifetime replacement policy for that part.

Yep. Just fill out the form linked to in my signature (below) and we'll get it take care of. Specify "91XG balun assembly".
